NURSE NAVIGATOR

Assists the Manager and Clinical Supervisor in the clinical practice as directed by the CAO/Manager and Clinical Supervisor. Able to fill in as medical assistant, triage, and scheduler and as needed and directed by the Manager and Clinical Supervisor. Collaborates with physicians and team members to accomplish tasks, collect data, and complete reports as directed.

• Accountable to the CAO and Clinic Manager.
• Assist with the IOD program; fill RX, obtain any prior auth, collect payments, submit reports to accounting dept.
• Documents patient care services by charting in the patient’s electronic medical record and other records.
• Wipes down all solid surfaces of exam rooms at the end of clinic every day.
• Answers the telephone or returns phone calls to patients taking care of the patient’s needs by either scheduling an appointment or answering patient and family questions.
• Guide patients and families through their healthcare experience, answer questions, provide education, helping them understand their diagnosis and treatment options.
• Empower patients through knowledge and emotional support to independently navigate through the patient care pathway. Reports off to the Manager of Clinical Services.
• Communicates problems with workflow, failure to understand directions or procedures, conflicts and other concerns encountered in the workday to the Manager and Director of Clinical Services. ?Collects and collates data as directed.
• Able to fill in as scheduler when needed. ?Able to fill in as triage when needed.
• Updates patient information in the UroChart system as they become aware that information has changed or is now available.
• Able to obtain pre-authorization specialty medication (Lupron, XGeva, Prolia, etc.)
• Accurately schedules patients for appointments as needed.
• Assists with re-scheduling of procedures and appointments as needed.

EDUCATION/CERTIFICATIONS:

• High School diploma or GED (General Education Diploma) required.
• Certified Nursing Assistant
